Report A Claim

If you need to report a claim, call Pilat Insurance at 440-888-1150 and a representative will help initiate the claim with the appropriate provider. The office is open M-F 8:30 am to 4:30 pm, closed Sat. and Sun. If you do not speak with a representative, please leave a message and we will contact you as soon as possible. 

You can also report a claim by directly calling the provider or filing your claim online. Here is a list of insurance providers, their phone number for filing a claim, and a link to their web page for filing a claim on-line.
